Middleware returns the early ingress gate. A nil policy is a structural disabled fast path: no client resolution, lookup, or policy evaluation occurs.

ResolveClientIP resolves a literal IP without DNS. Forwarding metadata is authoritative only when the immediate direct peer is trusted.
